Preparing for Your First Appointment

Congratulations! You have taken one of the most important steps to resolving your infertility issues by scheduling your first appointment to see a specialist. For many of you, this was a very difficult decision but one we are glad you made!

Here is information we hope you find useful as you prepare for your first appointment.

What should I do before I come to your office?

  • Please call your insurance provider to verify your infertility medical coverage.
  • If your insurance requires a referral please call your primary care physicians office.
  • If you have Aetna insurance, you must first register through their hotline prior to your first visit with us. You can do this by calling: 800-575-5999.
  • Please complete all new patient paperwork: patient questionnaire, demographics, email consent and medical release form.
  • Please have any old records either faxed to the office or bring them with you to your first appointment. 

How do I get to your offices?

We know this first appointment can be overwhelming, so we suggest you get directions and have a good idea of how long it will take you to get here. On the home page, we have several easy-to-use directional links to get you here from your home or office. Will my insurance cover my treatment? What will be my financial responsibilities?

Before your first visit, we suggest trying to become educated about what your insurance benefits will pay for infertility treatment. Here are a few phone calls you should make:

  • Please call your insurance provider to verify your infertility medical coverage.
  • If your insurance requires a referral, please call your primary care physician's office to obtain one.
  • If you have Aetna insurance, you must first register through their hot line at: 800-575-5999 prior to your first visit with us.

One of our financial specialists can help you make sense of what your insurance will pay or not pay. They will schedule a telephone appointment prior to your initial consultation where you can review benefits and ask questions.

What should I do if I have gone to another infertility specialist or my OB/GYN provided care?

We will need your records from your previous treatment. By knowing the results of diagnostic tests or the outcome of past treatments, we can save you precious time, effort and money. Please have any old records either faxed to the office if possible. You may also bring them along with you at your first office visit.

What should we expect during the first visit?

  • Please arrive 15 minutes prior to your scheduled time. Expect your visit to last 1 ½ hours.
  • Please bring your completed paperwork along with your driver’s license, insurance card and specialist co-pay.
  • Your partner is strongly encouraged to attend the new patient visit, if possible.
  • We will need both you and your partner's drivers license and insurance cards.
  • Your new patient visit will include: consultation with one of our physicians, a vaginal ultra sound and possible blood work. You will also be meeting with one of our nurses.
